What type of graphics card is this?

It's a passively cooled card. That big metal thing is the heat sink. The card is a low power card and the heat sink alone is able to cool it.

From case airflow, most cases even bottom bin OEM cases have 1 cpu fan and 1 case fan.

Even without direct airflow there is still heat transfer, it just isn't as fast as it would be with a fan.

The card in question probably uses very little power (because it's probably dirt slow), and thus needs very little actual airflow to remain within safe operating temperatures.

Its such a low powered POS card that it dosent need a fan, replace the heatsink with cheese and it'll likely remain cool. Reverse image search says its a Geforce GT610.

Great for HTPC or non gaming use though!

if you place a fan near that card temps will drop dramatically and you can make some good OC, at least it was the case with my passive geforce 7300.
