What is the setup required to create an online course?

Something like this: https://www.youtube.com/user/EducatorVids
(They have better resolution at educator.com but it's a paid site)

It's neat how the screen splits into three - the left is the 'table of content', the right shows the teacher with a tablet while the middle shows the screen that he's reading from (and annotating on). How's this done? The right screen is obviously a video feed, but how are we seeing the middle and left screen? Is that from some screen capture software or another video?

A different style is the crash course series by John Green:

These seem to be merge of two video feeds, right? It looks more engaging, but may not be suitable for technical subjects like trigonometry or programming, where the teacher has to show a computer screen or scribble on it?

I just took a quick look at the sites, the Educator video looks like a screen capture of a computer screen with an embedded video.

You can get the multiple video effect by dividing the video resolution and using multiple video tracks and mapped overlays. There may be other ways to do it as well.
