What is the worst physical pain you've ever endured?

My ears wouldn't pop when descending for landing on a plane once. They didn't pop for several more hours. Talk about pain.

An ear infection that led to the ear drum bursting. Fvck me if it wasn't the worst head pain I have ever experienced.

No, the worst pain, PERIOD.
 
After tearing my ACL, MCL, and meniscus in our Long Island Championship football game, I had to work from 3-1030 waiting tables (game had ended at 2 and I wound up playing for a decent amount in the game after tearing my knee up). As luck would have it, the other waiter didn't show up tonight, there is no busboy on Sundays, and it was a crowded dining room that night (at least 4 tables I was waiting on at any given time).

That night truly sucked.

VERY bad sunburn... jumped into a very cholorinated swimming pool. burned for ~5 hours after.. Learned my lesson (blistering sunburn.. i am an idiot)

No. It healed and was perfect again. I was 14 at teh time.
 
Been hit in the balls a few times and also got bad sunburn after a marathon. It was mostly my shoulders and upper back. I was peeling for about 2-3 weeks.
 
This one time I was running home and tripped and fell on the sidewalk and banged my shin pretty good.

Tore my ACL in half and tore the cartilage. I walked on it for 2 weeks before I went to the doctor. I finally went because my leg kept lock up on me at an extreme angle. They didn't even know I tore the ligament till they cut me open to fix the cartilage and found the other end of my ligament down in my calf.
 
I've broken a few bones, been hit with some pretty hard things, severely burned, and had acid splashed in my eyes.

I'd have to say the worst though was moving a recliner barefooted and the wood base scraping the skin and tonails off three of my toes. A close second was falling out of a tree and landing flat on my ass from about 20'.

Broke my neck, spent a week in traction and docs tigtening the bolts that were in my skull, had a spinal fusion that includes bone from my hips and around 10 titanium screws and 7 or 8 titanium plates holding a couple vertebrae together. I've posted my xrays here many times.

edit: OHH...I had chest surgery wide awake once. Novicane only.
 
Stress fracture in the femur. It was a sudden occurence during a 5 mile run for CC. All of a sudden I felt a sharp pain above my knee that wouldn't go away. The worst part was that I had to run 2 miles before I could get help. (Wasn't able to walk for a week after because of the pain)
 
I was climbing a tree when one of my vertebrae slipped and started putting pressure on my entire spinal cord.
