Fiscal conservative is right there in the poll. That implies you are conservative about only fiscal issues and moderate to liberal for social issues.
You can be fiscally conservative and socially liberal or you can be fiscally conservative and socially conservative. I don't see why you lump these groups together. Being fiscally conservative by no means implies that you are only conservative about fiscal issues.
Not sure why social conservative is on the list. I've never heard of a social conservative fiscal liberal before.
I think George Bush Jr. probably is pretty close to that mark (compassionate conservative was a common phrase of his where compassion is generally a fiscal liberal idea). And Bush Jr. certainly was freespending. In 2000 the US government spent $1.79T, in 2008 it spent $2.98T (a 66.7% increase in spending in 8 years is very liberal in spending). In comparison, Clinton increased spending 28.5% in his 8-year term, and Obama is on track to increase spending 34% in his 8 years but that may vary a bit since there is still time.
There isn't a great example in the US political parties. But this pretty much describes the typical Catholic voter.
