what do atheists say when you sneeze?

Na zdravlje (or similar in all other slav languages), gesundheit in kraut, etc. (both refer to one's health).

I don't say anything usually.

I ignore it usually, I think it's useless to say something every time someone sneezes.

Anyway in italian we say "salute" which means health so there isn't this problem.

Saying "Bless you" after a sneeze is a passive aggressive assault.
You are trying to force me to say "thank you".
I will not fall for your games!

I say "bless you" or "salud" depending on to whom I'm speaking. Both are tradition. Reading religious overtones into "bless you" is silly.

I don't get the bless you.

but society expects it and it does not hurt to be polite. you never know who is around.
