What causing Windows 10 desktop sync problem?

I had this problem for awhile esp waking up form hibernation where the display on my Windows 10 Pro 64bit I can see the desktop but and what looks like a different taskbar and most times I can open things on my desktop but they only appear when I use remote to show the desktop on my second HDTV.

At one point I thought this was being caused by my HDMI to DP 4kconnector but I recently replaced that with a actual HDMI-DP 4k cable.

I see that the problem is fixed once the 2 HDTVs are resynced so going to post this question for feedback as this is the best I can explain the problem for feedback thanks
 
I had to change my desktop display res to 4096x2160 from the recommended 3840x2160 as well as some monitor tweaks to get screen centered this was all new to me on setting up my 4k HDTV for the first time.

There both running 60hz as but I dont know if that setting would have the problem but glad I have the ability to run them at 60hz as opposed to 30hz anyways 🙂
