I my case, it was on the top shelf of the A/V rack, well ventilated, in a central cooling&heat home, and respectfully treated, as are all my electronic devices. So was my bro's, plus he used the nyko cooler from day 1. What percentage of failures involved user abuse, and which are strictly faulty design and/or hardware related, I don't know, but both undoubtedly exist.
I know of one personal case of a friend scratching a disk while turning it from side to vertical while it was on. Other than that all of us still have all our original 360s. I personally don't even think it's that loud, when I'm gaming the volumes up and I can't hear it for a damn. It's definitely louder than the newer ones for sure twice as loud atleast, but I hardly notice it when gaming or watching a movie on it. My 360 sits on part of my desk with my wii on top of it =) Never have any problems with either, it does occasionally get intesly hot in here and I have had it lock up on me 2 or 3 times, but never had any problems most people report having.
