Not entirely "airbrushing"... I would guess a masked semitransparent layer blurred and possibly cloning to smooth the skin, and a lot of very good selection skills and various transforms to move entire sections of the head (chin, hair, lips). I'm sure some different brushes were used to "paint" portions of the image, but that's not how all of it was done.

Nitpicking the description: a "long lens" won't make the hair appear unusually high; the angle that the photographer used will. Non-long (hence wide) lenses are not often used in portrature due to the much worse distortions that they cause in facial features.
