And? Depends on the context of the shot...and what the company wants. Not everything has to be 100% noise, grain or artifact free.
 
As it was mention, sometimes high noise is favored when trying to create a certain shot. In this situation, the higher noise makes the photo look a bit older.

Yes they did, and I think it is a generic old film/paper effect. Increase contrast with slight reticulation possibly a some cross processing effect.
 
Yeah, people have been adding grain to photos, including a lot of 'glamour' photos, for about as long as there was grain to add, and far before it was noise.
