Virtual memory is low - have tried almost everything to fix it

I have a client who has had the "Virtual memory is low" issue forever. We have tried the following fixes:

Replaced RAM (stick was bad)
Replaced hard drive with a fresh install of the OS
Made sure plenty of space was available on the hard drive (50GB free)
Scanned for spyware
Scanned for viruses
Checked for programs hogging a lot of memory (found none)

Also changed the Virtual Memory settings via these instructions (from pcmag.com):

Right-click My Computer and choose Properties. Click the Advanced tab. Click the Settings button in the Performance panel. Click the Advanced tab in the Performance dialog. Click the Change button in the Virtual memory panel. Whew! This setting is buried quite thoroughly! Look for the System managed size option, select it if it isn't already selected, and then click OK, OK, OK. That should end the warnings.

Is there such thing as a motherboard having a memory leak?

Any experience with Earthlink's Total Access causing this?

What would you suggest trying next?

The computer has also started to freeze, even as it sits overnight when it is not being used. Scanned the hard drive for bad sectors. It passed. Suggestions?

FYI: She said that soon after I replaced the hard drive and RAM, and did a fresh install of the OS, it started to give the Virtual Memory error (maybe a few days after, or up to a week. Also, she has a 2.5GHz system with 512MB RAM, and she was rebooting the computer once a week. I have advised her to reboot it every 2-3 of days.
