Vert Sync.

always off. something to do with refresh timings. im not sure exactly but i know it's a hog on frame rate. disabling it gave me 40 more fps in half life. always keep that thing off (although you may witness minor tearing or clipping, you usually wont notice it).
 
vsync simply matches the frame rate of a game to your vertical refresh rate. So if your refresh rate is 85Hz, then the game will try and run at 85 FPS, it creates a unison between the two. However, vsync is often the cause of low frames since it's limited to the rate.

from what I've read Vsync is bad because it will try to match the frame rate to the refresh rate and if your FPS drops below your refresh rate it chops your framerate in half.

For example, if you run at 1024x768 at 85Hz and your framerate is usually 100 fps, you should be fine. However, if your framerate ever dips below 85 fps (for instance in CS when someone throws a smoke bomb), your video card will drop you to 40 fps and you'll see a little tearing.
