Silly me for not posting this little find. I actually did this a few days ago when I processed all my Presidents Day rebate forms. The recent OfficeMax receipts clearly state: Enter/Win www.officemax.com/store/survery.

I submitted 4 surverys (and 4 follow-up surverys). The initial survey gives you a chance of winning $50 and $100 OM GC's. The follow-up survey is a guaranteed $10 GC; however, based on what they claim, you'll only receive one of those. I'm waiting to see what they send me for my 4 follow-up surveys. Perhaps I'll get four $10 GC's!

BTW, these were my 4 receipts:
[*]#1: Purchase of upcoming Prez Day sales items 2 days before the sale to "secure the merchandise."
[*]#2: Refund of all the Prez Day sales items (at the PRE-SALES PRICEZ) during Prez Day sale.
[*]#3: Re-purchase of Prez Day sales items during Prez Day sale (to get the Prez Day Prices & rebate receipts showing the correct purchase date).
[*]#4: Pacific Ditial 52x CD-RW additional Prez Day purchase ($29.99 - $20MIR). I found out (after Step #3) that my local OM was substituting it for the Verbatim 48x CD-RW. (I wasn't interested in the 48x CD-RW so it wasn't part of my Step #1 purchases). $10AR for a 52x CD-RW w/ a $30 up-front price works for me!

How'd you get a mouse? It says "Excludes electronics, Gift Cards, PDAs, cameras, phones, computers" on the bottom.

I don't think you actually have to have a reciept from OM. All you need to do is put random numbers in for the transaction, date, and store number. I wanted to do it with an online order so for the store number I put "Online" and I still got to the page where they said they would send me the $10 gc. I don't know if they will for sure, but to make this deal "hotter" you don't even need to have a receipt.

The offer for the $10 coupon still says Offer ends 3-30 but the drawing to win either one $100 OfficeMax gift card or one of four $50 OfficeMax gift cards might be still good.
