unlocked phone working fine. factory reset and now phone is locked :(

i have an old lg 70 metroc pcs phone. it was unlocked from metro pcs.
with an h2o wireless sim, the phone was working fine. but since i was giving the phone to mom, i factory reset the phone. now, the phone refuses to recognize the sim "no sim card" with a warning on the home screen "Insert metro pcs sim. emergency calls only."
i didnt know a factory reset would wipe out unlocked settings. i still have the unlocked code metro pcs gave me. so i entered it again but lg phone gives out this error: "your phone is not personalized" weird.

Can you use an apn?

i manually added an apn for h2owireless. saved it. but when i look at the apn info again, the phone said no apn. weird.
