We tend to use either steam or battlelog. We did the Hamachi/vent combo few years back but that was a bit annoying.

Dolby Axon has become my favorite voice program. It doesn't require a server but lets you make permanent channels. The sound quality is very good too. In particular I have a friend who uses a crappy mic with a ton of background noise. On Skype I get constant static into my headset. With Axon there's none.
