The one that bothers me the most is the one that happens right after a map switch. The thing stops, loads normally (LOADING pops up) and then out of the blue it flips back to the menu (like the one after you hit Esc in-game) and out comes a Windows error window saying /valve/halflife.wad could not be found!!! Bitch!

The second one is when I Esc out, and then try to get back in. The playing screen is squished into the top 1/3rd of the display, and it's all purple lines! (somewhat horizontal, with a diagonal tilt)
