trouble recovering RAID5

I have a RAID5 setup at home 4x160GB
one of the drives died or seemingly... it was out of the array. Took "dead" HD out of the machine, put into another windows machine which detected and formatted it n/p

So I stuck back intot he RAID controller card and entered RAID setup. the existing array only shows 3 disks with 1 reported as missing or corrupt.
If I go to drive assignment the card sees the 4 disk but will not let me allocate it to the exisitng RAID5 array... any advice?

Any chance it's a bad data cable? Or maybe the controller's getting ready to kick the bucket?
