Solved! Toshiba flash drive stuck with Logitech webcam icon.

Drives take on icons, if they have certain files in the root directory. Look for .ICO or .INI files, and try renaming them. Was the flash drive used to hold logitech drivers, copied from an install CD, at any time? That's probably where they came from.
 
Almost assuredly it had an autorun.ini, with an autorun.exe and icon in it at some point. That's what establishes the icon on removable media.
 
There are no files on the drive. I have formatted the drive many times, used Windows/ EaseUs / MiniTools partiton manager to delete partition/format the drive. None helped.
 
In Windows Explorer, right click on the drive, then check properties. Look for the driver that has that icon. That may tell you where it is coming from.
 
One way to go around this is to use the autorun.inf method described here.

It's the same method used by MS to change the icon of a flash drive that contains a Windows installation kit. Example bellow.
