I think those would be the result of drunk technicians, engineers, or managers. Once the rocket's ignited, I don't think the crew can flip the "blow up" switch.

From what I've read, some of the problems over the years have happened because low-level engineers' warnings to their superiors were not addressed, or simply ignored.

To the "isn't it all automatic" comments, the majority of take off activities are indeed automated of the crew only the first and second chairs really have any work to do.

That said there are an incredible amount of contingencie plans in case the launch fails (e.g. en engine fails in flight, emergency escape before launch, etc) and being drunk would certainly not the crews reaction speed if they evacuated or had to jump out (yep, there is plans for even that...)
