We start at 15% and adjust from there. VERY good service, up to 25%...mediocre service, down to 10%...crappy service, 2 pennies. That way, the server understands that we just "forgot" to tip, and that we plain were dis-satisfied with the service we received. and YES, I'm one of those people who holds the server responsible for the quality that comes out of the kitchen. It's their job to ensure that things are prepared properly before bringing it to my table, AND, their responsibility to make sure drinks (be it coffee, Iced Tea, soda, beer, etc>) never runs out, or at least asks if we would like another...
I have NO problem tipping well, but I insist on good service to earn it...a gratuity is optional, NOT required...

I generally don't go below 10% unless it's extremely bad, but if the service is exceptional I'll go above 20.

same as above but if its real bad then the ole stand-by of .25 since i dont carry change cept quaters now.

 
10 percent or a little less for bad service. If the service is bad, i don't spend too much time doing the math...

15 percent or more on average

20 percent or more if i'm really pleased
