The incident was likely boiling over for a while up to that point (As these things often do) Plenty of time for cameraman to set up and start filming, most likely.

Doesn't look like he was focusing on samaritan, but it was wide enough to get him in the shot.
Plus, If we was being smart and keeping one eye open and one on the viewfinder, he would have noticed the guy getting up, which would prompt him to include him in the shot.
