Simply put: go to the wed site, check out the tribute, print Test drive paper and Voila!!

P.S. You don't actually have to test drive the car just ask the dealer if they'll be kind to fill it out. I've done this four times now and it works like a charm. Enjoy.

I requesteded a "Tribute" Brochure from Mazda
A week or 2 later I got an e-mail from them with a link to print the certificate.
YMMV.
