I was supposed to send a form in with my 1040EZ (i belive that is what it was). The form is a form 8453-OL. I am supoosed to send it somewhere, but I can't figure out. Can anyone help me?
 
you filed electronically so you send the 8453 to the IRS to tell them you really did file electronically

search IRS web site and it will tell you where to send it to
