I've been having some problems with my computer recently (mostly when gaming) with Artifacts, freezing, etc..
I figured the problem was the video card because it's been over heating with the gpu fan reaching 100% occasionally. Temps have been hovering around 85c. Peaking at 90c under load. Other times however, it's stays below 60c when playing the same game. Sometimes I idle at 40c. Other times it idles at 60c. Which seems odd.
I was told to check the +12v reading in bios, which show a normal level at +12.09.
However, when I checked the readings using speedfan it shows two +12v readings. One is normal but the other is above 16v! Obviously that's too high!
So how trustworthy are speedfan readings? Should I replace my power supply?
Specs (I know its ancient. Please no judgement
My plan is to upgrade everything this fall.)

Phenom II X4 940 Black Edition
ASUS M4N82 Deluxe mobo
8 Gig ram (4x2)
Corsair CMPSU-650TX 650-Watt
Antec 900 case
Win10 home 64 bit
Air flow in the case is good and everything was air blown for dust about a month ago.
