It's hard to turn on the insurance and pharmaceutical companies that help finance your reelection campaigns knowing that if you do they will become your mortal enemies and will heavily finance your opposition in the next election. That's part of the problem.

Note that it may be impossible to find a single middle class person in another first world nation that has socialized medicine who would want to scrap their system in exchange for the American system.
