I don't know how many problems you practice on a regular basis, but it's apparently not enough. You have to understand how the test preparers think. Given your current confusion over the english portion, you have to do a lot more. From what I can tell, you need to do at least a few more thousand practice problems to even be comfortable with the exam. If you think I am exaggerating about doing at least a few thousand more problems, then you have a completely different problem as well.
As for math, you just need to slow down a bit.
