That's nothing. I got an enlargement email and they showed before and after pics. Somehow, the white guy turned black. I'm sold.

String the lottery people along for a while... See if you can get them to do something amusing that you can post here later on. No harm in sending the form back to him with a fake name... For the fake income, put a pretty significant income in - 300k. Of course, if they need you to travel somewhere, you can't, because you're on the list of international terrorism suspects! :p And, he'll have to travel to meet you! You also can't send cash because you suspect they're monitoring you incoming and outgoing mail and will confiscate anything like that because they're attempting to freeze your assets.
