SMC not formatting

I took my smart media card out of my mp3 player (an oldie but a goodie: Rio500), and put it into my card reader and I was unable to format it so that the card reader could read it. I tried the MS-DOS command line utility, and also the GUI utility, but none work. I don't believe that I damaged, the card, so what could be wrong? Thanks in advance.
