Slimline form-factor video card that would play GTA4

I'm looking for a video-card that would fit in a slimline form factor PC, that has HDMI output, and would be able to play GTA4 (say, 786x1280, 30 fps at medium detail), what card do I need for this?
 
ATI has slim form factor 4850s, that'll probably be about the fastest video card you can find in that form factor.
