I delete everything out of startmenu that programs add and copy a shortcut to the program (if I plan to use it more than once a week) to one of 3 shortcut bars across the top and right sides of my screen.

yes I am on a CRT. It's obviously not enabled as a default and I didn't like it since I was used to cleartype by now... always on CRTs.
 
AVG loads on startup, and nothing else. If something installs itself as auto start I will either remove the program entirely or remove its autostart option depending on the usefulness of the program.
