We use an industry-standard form (medical practice). The section is clearly listed as optional in bold print. We have no use for the information at all. It's just a standard form.
If the question is optional, then it has no bearing on her application and shouldn't even be brought into question. What she chooses to put down is also inconsequential.
But to answer your question, it sounds to me like she was trying to have a little fun. A good sense of humor is always a good thing. So yeah, I'd hire her if she's the most qualified person for the job.
