Under the "Ethnic Group" section, the choices are: Caucasian, Black, Hispanic, Asian/Pac, Native American, and Other. She checked the "Other" box and under the box wrote this. "1/2 Check, 1/2 Mexican, 1/2 Hispanic."

We use an industry-standard form (medical practice). The section is clearly listed as optional in bold print. We have no use for the information at all. It's just a standard form.
