N64 game called Shadow Man. The whole thing was really creepy and serial killery, but the worse was in the Asylum when pig-headed, hook-handed motherfuckers would jump out at you.
Hotel level from Vampire was pretty scary overall.

stupid now but back in resident evil 1, walking down a hallway and the stupid dogs crashed thru the windows to attack you, first time EVER i dropped my controller gaming LOL
I remember when playing Shadow of Chernobyl I was leaving the Agroprom Underground and was tense as hell after all the crap that was down there. I went into a tunnel, turned right and headed for the exit ladder when I hear a large moan. I frantically turn around only to see a controller, that I must have missed on the left standing right behind me. It was the only time I had ever yelled during a video game.
This is essentially the scariest moment in any game I've ever played. I was playing the game after midnight when I came across this room in Silent Hill 4, saw that fucking thing, then almost immediately turned off the game and went to bed.

Unreal 1. Exiting first area through stone hallway when both ends got blocked and the lights went off. When the creature jumped out from the hidden compartment I just about messed myself. I was finger fumbling the keys and couldn't aim the mouse straight. Got me good.
FEAR, where that little girl runs in front of you that first time. Gave me the biggest creepiest chills ever, nothing scary about the rest of the game though.
That was a good one. Light clicking off one-by-one.

It was probably just my sleep deprivation, but I was cowering under a staircase in the game trying to work my nerve up enough to go and see what happened.
