The last time I've been to one was back in 1999 or 2000 on 34th street, near the Empire State Building. The midget from Games of Thrones was there with 2 tall blondes.

Don't remember what I ordered. I do remember going there again and seeing a roach run past a pizza on display. I still bought a slice anyway, I didn't give a crap.
 
They were in bankruptcy last year. I never bought their pizza, too overpriced even for mall food (the burger fast food joints are cheaper).

There are some in the airports, particularly in PHL airport, I believe. Overpriced (even by airport standards) imitation of Italian food.
 
We have one at the mall. Their pizza sucks in general, but their spinach calzones are pretty damned delicious.
