They employ financial counselors who help patients and patient families navigate through the tangle of medicare, medicaid, insurance, income and payment plans. They often will charge on a sliding scale based on what a person can afford, and often will accept payment in pieces.

If you go to an emergency room with injuries, etc that are immediately life-threatening, you will be treated. I have a friend who was T-boned by a truck in Montana, critically-injured, life-flighted from Montana to a trauma ward in Seattle (because it was the closest hospital that was equipped to deal with his injuries), and spent several weeks recovering in the intensive care unit. Hugely expensive. He couldn't afford to pay and did not have insurance.
The hospital still provided care and he's alive and well now. I believe that the Hospital forgave all or almost all of the debt.
Despite the rhetoric of those preaching for socialized health care, the stories about people being refused critical treatments because they can't pay are simply myths. If you're dying and you show up in an emergency room, you're going to get treated whether you can pay or not.
