Long story short I ordered a bluetooth headset on sale and it came and was defective. I was told to send it back via them sending UPS to my house to pick up the package.

Now, do I need to print a shipping label and affix it to the box or will the UPS dude take care of all that?
 
you need to put some type of shipping label. From my experience, after taking with a rep, they gave me a printable link for a label to affix.
