Rebate address for Microsoft Natural Keyboard/Wheelmouse combo?

This seems the more appropriate forum for this question than Off Topic, but I find the decisions on that rather arbitrary around here. Anyway...

I bought this at Best Buy awhile back for $30 with a $20 rebate, but the rebate information doesn't have an address on it!! Does anyone know where I can look for this info? MS' website didn't have it either that I could find (really can't find much on that website, actually!).
