I concur. (a) is rather rude, (b) is not. Normally though, I choose (c) Have money in hand, be patient enough to wait the five seconds for him/her to finish ringing it up and then extend palm to receive $.

WTF? If you give a cashier money WHILE they are scanning your items, that's completely retarded. How do you know how much money to give them before they are done scanning?
