the only mmo i've played was conan. obviously, it dominates the "worst mmo" category.

The sad thing about AoC was the first 20 levels of it were really cool. I think more MMO's should stick to a story while you level and then make the world a huge open persistent one when you max out (the story would be designed so you max by the end of it). That way you wouldn't end up with all of these wasted areas for noobies a couple months after release and you could focus on making the open world a place for all the max level people to explore.

1) Everquest (Immersive)
2) Asheron's Call (Super cool concepts)
3) Vanguard (Pretty solid today)
4) Aion (pretty fun once you lvl up and rift/do spy quests)
5) Everquest 2 (Solid but a bit stale on environments/gameplay)

Worst

1) Age of Conan (Glitchy and stale gameplay)
2) Dark Age of Camelot (Too Linear Progression)
3) Asheron's Call 2 (Might have been good if it didnt have Asheron's Call in the name)
4) WoW (Too Linear and stale environments/gameplay)
5) Runes of Magic (Glitchy and stale gameplay)

I played/beta a few more but those top the list.

I've played several, though many of them were in beta or free trials.

Anyway:

Best:

1. LOTRO
2. WoW

Worst:

1. Pirates of the Burning Sea (just a giant, flaming bag of dog business)
2. AoC
3. Vanguard (I actually enjoyed some parts of this, but it was too buggy and annoying.

I'm sure I've played more, but I can't remember them right now.

Best:

1 - PSO:BB (not really a MMO similar to Guild Wars and DDO, most fun i ever had online)
2 - The Chronicles of Spellborn (love the concept and lore, best combat system i've experience in a MMO, skill based not gear based)
3 - Ferentus (game had a awesome feel to it, shame it never made it out of beta)
4 - Granado Espada/Sword of the New World (controlling 3 characters at once was different)

the first 20 levels were all that were completed. that, and the software was terribad.

Haven't played that many to give you the full 5 list of each category... so...

Best:
1. Final Fantasy XI (still playing)
2. Eve Online (gave my accounts to friend who still plays it)
3. DAoC, pre-Frontiers expansion. First MMO and loved it.

Worst:
1. Star Wars Galaxy. (played this like 1 day and trashed it. What a waste of money)

I think they are all good games in their own way just but most are just time sinks and traveling to x to mission y then travel to w to mission u. The whole traveling around in mmo's takes more time then actually doing the quests which is what is keeping me away from them these days, though I do miss the group action.

Anarchy Online deserved better, but the launch stability issues turned off so many people it was doomed to semi obscurity, same thing happened to Vanguard imo

Guild Wars is fine because its free. All others need not apply.
