Having been a dealership service manager for a long time I've seen many things. You must drive with the customer who claims brake warpage every 20 thousand miles. I would sit in the passenger seat and just watch the driver. Often this can reveal many things for example.
Someone above mentioned a person using two feet, left to brake and right for gas, and then often having both down at the same time to some degree.
There is also the case of the wide footed woman, who was able to move from brake to gas pedal (pivoting their foot) while still holding down the brake pedal. Her brakes wore out in half the time and she also cooked the rotors at the same time. Quite often a handicapped person with the foot or leg problem will have the same result.
I saw another type of driver who would drive normally and then on the drive back to the dealership, he swirved quickly to hit a puddle of water on the side of the road. I asked him about this and he said that he was cleaning his undercarriage with the quick force of water. I also mentioned to him that he is also drenching his hot rotors with cold water causing them to warp.
If it was not so late, I am sure I can come up with other scenarios for warped rotors or quickly worn out brakes.
