This is strange, I have a Athlon XP 1700 on a Epox 8KHA+ with 512mb Crucial DDR maxed out and a Gainward Geforce 3 @ 225/500 using the drivers from Nvidia's web site 21.83 I thin. I have vsync disabled in the nvidia settings and sync every frame truned off in Quake 3 options.Any idea what up here ?

I guess you have installed the 4-in-1's? Also check to make sure AA is turned off.

/cg_maxfps 125 or whatever you want to be maxed. I am using your exact setup and dont dip below 100 at all. You dont have anti aliasing on do you? Manually set it off. 21.83 drivers work well for me. Do you have 4x on in the bios?

<< com_maxfps 0 will remove your framerate cap. >>

Holy crap, I now get 235-250 fps, sometimes going as low as 185.

where do you type the com_maxfps 0 command? Is it just on the screen you get when you hit the toggle bar? (command prompt??)

Set it in your q3config file (located in your baseq3 file). Open it with notepad, then do Edit and find: seta com_maxfps "0"

Yeah, ain't it cool? My Ti500 often hits 300 FPS. :)

Hit tilde (~).
