This is a self executeable Flash presentation by Flightsafety Intl. It shows the events leading up to and taking place during the incident.

Basically, an airplane gets lost in near zero vis, and we almost get a lot of noise pollution. 😉

Congrats to USA2998 for not taking chances. 🙂

Sorry about it being exe, nothing I can do about that.

Ok, well at first I thought it was all the pilots fault for not knowing where the the hell they were, but then the controller proves she doesn't understand what United is telling her. She also keeps discounting what United is telling her and keeps thinking they are still lost when they are looking at the signs.

I would say the Pilots were the cause at first, but the Controller aggravated the situation and has some blame.

I would say 60/40 or 50/50 to split the blame with United taking more of the blame. You have to think the controller should have verified with the United before she let the Fedex plane take off.

BTW, I accidently voted for the Pliot. Put me down as both...

Well, I see no fault on her part for clearing the Fedex. It was United's job to stay on November, and to properly report their position.

Now, when 1448 reported at Kilo, she should've noticed it. Theres only one kilo, and it crosses 5R/23L. You NEVER clear ANYTHING when in doubt.

The fact that she cleared the usair for t/o TWICE is just mind boggling.

Yeah, but don't you think in Fog and 1/4 mile visibility you should double check sometimes? 😉 Hey, just my opinion. Personally I won't fly if it is foggy out. Already lost my father because of that.

She shouldn't have had to.

If you dont mind me asking, was it a plane crash? Which one?
