Problems with 9700 Pro holding its overclock

I have a graphics card that will not keep an overclock. One day it will be fine at a particular setting, the next it will have problems with that same setting. I used to have a 9800 Pro, and that never had that problem, so I'm pretty sure it's not something else in my system causing the problem. As you can imagine, this is extremely frustrating.

The 9700 Pro has its shim removed and a modded P4 aluminum heatsink bolted to it, and a 80mm YS Tech fan at 50CFM/40dB. I have also volt modded the GPU. I've tried 400MHz core, that didn't work. 395 was good one day, but the next it had artifacts in Halo, so I bumped it down to 390, which worked perfectly. Now 390 has artifacts. My memory does 350MHz (even more according to ATITool) which is the max of the Samsung 2.8ns chips, and that will be glitchy usually when I first turn on the system. After I restart it however, it will work fine.

I've had multiple suggestions, such as unstable rails on my power supply. I have a Thermaltake 420W, and the rails are within 1% margin. Rails go as follows: +3.3v = 30A; +5v = 40A; +12v = 18A.

I've also had someone suggest that my card is overheating, and that I'm overclocking it too far, causing it to strain and damage the card, making my overclock get lower and lower, until I ruin it. I don't like that scenario, but it almost sounds like what's happening. To be safe, I'm running it at stock right now.

Is there any hope for it?

Same thing happened to my 9700 Pro. I ran it at 350/350 for a long time, then I stopped using it. When I started using it again, it could no longer run those settings. In fact if I try to overclock it at all now, it gets f'ed up and I have to drop back to stock. Can't even do 333/333 according to atitool anymore. /Shrug it's still a decent card at stock I guess.
