Problem with video display upon boot-up

I've finally saved enough to put together my dream machine. The only problem is that upon boot-up, the screen is garbled (you can see the normal boot information, but there is extra stuff all over the screen).

Here are my specs:
AMD XP 2100
Epox 8K3A motherboard
Corsair XMS2700 DDR memory (256 Mb)
Gainward GeForce 4 Ti4600 video card
Samsung SyncMaster 955DF monitor

Now, here is one possible problem area - the Gainward has 2 DVI connectors and one S-Video, but my monitor does not accept either. The card came with an adapter, though.

(Yes, this is the first time I've booted up (actually, the first 8-9 times I've booted my new system))

Any thoughts would be appreciated!
 
Yes, I've tried both outputs.

I have just re-seated my memory and video card and will try that.

I'm at a complete loss (and am hoping it doesn't mean a problem with either my motherboard or video card) 🙁
