edit: most of the devices with interfaces in an operating room, if used incorrectly, can lead to misinformation or unwanted actions, and easily kill the patient.

Too vague, since there are certainly things I can think of that could happen, it is not 'false'.

So 'true' I guess.

A vague question - you don't really say what you mean by 'user interface'. The interface doesn't have to be electronic/computerised to be dangerous.

However, I could give a few examples if you need them.

Someone at work brought in an article about some software implementation at a hospital that was trying to bring them more into the digital age and the death rate of their patients went up because of typos and poorly designed software.

Sure, put the "keep patient alive" button right next to the "make patient die" button, and then label them poorly.

True, it's happened before. Sometimes the nurses get so good at entering info into old user interfaces, that they work faster than the interface can allow. This can lead to information to be entered incorrectly.

The Baxter Colleague infusion pumps at our hospital were supposed to be recalled (but there werent any viable alternatives). The "start" button is right next to the "on/off" button. Oops.
