Someone at work brought in an article about some software implementation at a hospital that was trying to bring them more into the digital age and the death rate of their patients went up because of typos and poorly designed software.
True, it's happened before. Sometimes the nurses get so good at entering info into old user interfaces, that they work faster than the interface can allow. This can lead to information to be entered incorrectly.
The Baxter Colleague infusion pumps at our hospital were supposed to be recalled (but there werent any viable alternatives). The "start" button is right next to the "on/off" button. Oops.
