I'm supposed to but I ran out and I'm too lazy (read: don't give a sh!t) to order more, so I just use my wife's cards at the moment.
 
I've got an assortment of them. Most often I'll use either the one from the production company or the distribution company, but I've also got others that don't have my name on them for other businesses like the grip & electric company, the special effects/stunt company, and the dojo.

I should get some made that are just me personally, but I've never done that.

Visually, my favorite is the one for the distribution company, which I made from a picture of the mountains to the west of me composited with the rainbow from another picture I'd taken on a different day.
