Heres what to do if you have free time and energy and they owe you more than say $50:
fill out the form, giving only the info you are comfortable with
-attach supporting documentation of your claim amount
-be as truthful as possible, as long as you are not INTENTIONALLY MISREPRESENTING the facts you are ok...dont get scared over the terminology
THEN FORGET ABOUT IT...
if you ever get any further documentation (or even a check) consider it a WIN. if you never hear about it again, nothing is lost....we will likely see nothing at all.
