pny 6800 rebate question (200 dollar deal from BF)

for those of you who have sent in the rebate (which is due dec 31), what did you put for the UPC? Do you just tear that one sticker off near the bottom of the card?
 
The box has a proof of purchase seal on it that I cut out and sent in. The upc on the card says that the warranty is void if removed. So they denied my rebate and I had to call them and explain the whole thing. She told me to resubmit it and they would honor it. I figured something like this would happen when I saw that the upc was actually on the card and not the box.
