Note 3. You can see at the top where it looks like a screen protector wasn't properly applied and there's bubbling, but it's not actually that. If you see it in person, it's actually a bright silver and it looks like the top layer is starting to peel (and crease like tinfoil). It used to only happen at the top left corner (presumably after a drop) but I dropped it again and now the whole top looks like this. It looks like it's slowly spreading (even when it was just the corner). Is this common and a prone issue?

I do have a screen protector on it but this started over a year later and it is discoloring even where the protector isn't (like over the earpiece & camera). I switched to this minimal case so I guess I got what I asked for...

Might just be the "chrome" paint peeling off or wearing down. I don't have a Samsung phone, but that's common on anything using paint or anodization rather than bare metal or plastic. Gloss clear coats can only go so far in protecting the paint.

It's the glass separating from the adhesive.
