I think what happened is that she had subconsciously decided to break it off but didn't have the guts to do it. She knew telling her bf that she slept with another guy would trigger a breakup, but this way she didn't have to say the words.

It's like the married guy who starts going to lunch with his secretary, then working late, then stopping for a drink after work, then one day they sleep together. He goes home thinking, "I don't know how that happened." It's a defense mechanism to avoid feeling like there was an actual plan to do a bad thing.
